M.D.ga xodim, u bajarayotgan ish haqidagi ma&#8217;- lumotlar kiritiladi. Jazolar M.D.ga yozilmaydi. Mehnat shartnomasi (kon- trakti) xodimning tashabbusi b-n ka- salligi, nogironligi, qarilik tufayli pensiyaga ketishi, oliy yoki o&#8217;rta maxsus o&#8217;quv yurtiga yoxud aspiranturaga kir- ganligi va qonun muayyan imtiyozlar, af- zalliklar berishni nazarda tutadigan boshqa sabablarga ko&#8217;ra bekor qilinganda, M.D.ga ishdan bo&#8217;shash haqidagi yozuv ushbu sabablarni ko&#8217;rsatgan holda yoziladi.
